feat(api): normalize model providers for picker and agent creation\n\n- Normalize models.list() output to include provider_name\n- Infer provider from model id when missing (openai/anthropic/google_ai/together/letta)\n- Apply similar normalization for embedding models\n\nThis fixes invalid "undefined/<model>" selections and 400 errors on agent create.
